Page 1 of a Life History form.
Lisa's answers state that she does not have a psychiatric/institutional history, has never been institutionalized, has never been connected to a squirrel group, has never been connected to a suppressive group, has never been connected to anyone not in good standing with the church, has never herself been out of good standing with the church, has never been declared an SP, has never blown from the Sea Org, has never had doubt about being in Scientology or the Sea Org, has never been offloaded from the Sea Org, and has never been dismissed from Scientology staff.
Questions 3 and 8 are very similar questions about connection to squirrel groups.

Lisa's answers state that she has never been a news reporter, is not closely connected to anyone "of known antagonism to mental or spiritual treatment or Scientology," has no criminal record, has never threated to "sue or embarass or attack Scientology or Scientologists" and has never been a party to such a thing, doesn't think an auditor or a Scientology book is responsible for any problems in her life.
Her answers say her auditing is done on her own determinism, she's gotten gains from Scientology, she does NOT "have an open mind with no personal hopes or desires for auditing or knowingness," she does feel anyone and anything can get better, she hasn't judged Scientology or Scientologists, she does have family or friends who have "expressed skepticism or been critical of Scientology", and she's not associated with anyone who's attacked Scientology.
Finally, her answers say that she's never taken LSD or angel dust, she has had "a full DRD declare" and attested to completing the Purification Rundown, she has never sold drugs, and she's never taken drugs since being in Scientology. (This last question doesn't differentiate between legal and illegal drugs.)

Lisa's answers state that she has never threatened or attempted suicide, is not an active drug dealer, is not connected to intelligence agencies (and hasn't been in the past and neither has anyone in her family), and is not in Scientology to get a story for the media or to disrupt the org.
According to her answers, she is married to Gene [Eugene J.] Skonetski, her spouse doesn't object to her working in the org and hasn't criticized Scientology, she has no problems with her ex-husband, she was divorced before due to incompatibility, and she has no children.
Finally, her answers say that her mother has expressed objections to Scientology, but doesn't do so currently, and that her father has not done so.

Lisa's answers state that none of her relatives has objected to Scientology or her working for the church, that she has had marital or family troubles, that she doesn't earn money from any source other than the church, that no one she's connected to has objected to Scientology, that none of her family or close friends have been associated with a government agency, that she's been in Scientology for 6 years, that she doesn't have anything she wants to deal with before joining staff, and that she does have debts.
