Eckard's Drug Co. record of Minkoff prescription

[TIF 5078 is an 8.5x11 which is apparently a photocopy of a smaller
4x6 page with Eckerd Drug Company Letterhead. The Eckerd form contains
a copy of both the prescription label for the pill bottle, as well as
Dr. Minkoff's original prescription for 30 500mg Chloral Hydrate
capsules]
